This presentation is an overview on how cardiac rehabilitation (CR) programs can incorporate practical strategies to implement Bundle Care within their services. Concepts of population health management and value based care with lean six sigma methodology will provide a framework to the strategies. The implementation of Bundle Care will be directed to CR programs who were selected for the incentivized vs non-incentivized models with strategies that will build scalable and sustainable healthcare bundles.
