Description: This presentation will review the patient group orientation process from two unique perspectives through the lens of value-based care. During this session, attendees will learn about the rational for implementing group orientation, different models to deliver group orientation, and data and techniques for data collection as it relates to group orientation. This presentation will relate their data to the value based care model of increased health care utilization with a decrease in time to enrollment while decreasing health care costs through efficiency.
