Description: This session introduces HWC as a powerful adjunct to cardiopulmonary rehabilitation—helping clinicians engage patients, sustain behavior change, and extend the benefits of rehab beyond the clinic. Participants will learn the foundations of HWC, its evidence base across chronic diseases, and approaches to integrating it into healthcare systems. Through case studies and practical examples, the session will demonstrate how HWC enhances self-efficacy, adherence, and quality of life, while reducing burden for patients, their informal caregivers, and cardiopulmonary rehabilitation care teams.
Learning Objectives:
- Explain the principles of Health and Wellness Coaching and how they align with cardiopulmonary rehabilitation goals.
- Assess the evidence supporting HWC in chronic disease populations, including its impact on adherence to care plans, clinical outcomes, and quality of life.
- Apply practical strategies to incorporate HWC techniques into rehabilitation practice to enhance patient engagement and long-term self-care.
